This needs to use the following tech:

amd module for javascript
mustache template + renderer for lazy load markup

lazy load setting needs to be visible but possibly with a warning that it is a bad idea to turn it off. upgrade.php script needs to turn it on by default for people upgrading from the old oembed filter.
