I have not tested how broad this issue is. The steps to reproduce below are based on my current setup in the Singleplayer world. The specific machinery may just be any GT block supporting it.
Place an LV Extruder and place a Basic Item Holder cover on the top.
Fill the Item Holder with extruder shapes (8 in my case)
Break the Extruder and pick it up.
Place the Extruder again and open the GUI.
Attempt to open the item cover while inside the Extruder GUI. The game crashes with the attached crashlog.
Some basic testing I've tried:
You do not need to place the extruder in a chest.
You do not need to log out of the world between breaking and placing it.
Opening the item holder cover directly (Shift + RMB) no longer caused a crash when continuing with step 5.
I have searched this issue tracker and there is nothing similar already. Posting on a closed issue saying the crash still exists will prompt us to investigate and reopen it once we confirm your report.
I can reproduce this crash consistently by following the exact steps I described above.
