Unable to Login page reformatting

Description

Work off of branch hi/show-qr-code.

After scanning the QR code, the redirect tries to login in the user. If they fail to login, they get taken to this Unable to Login page. We need to fix the styling and contents on this page.

• Follow fonts/styling from the other pages.
• ‘Uh oh, looks like we weren’t able to log you in.’ is sufficient, but feel free to add other text as you see fit!
• Also add a manual login button.
