<li>各項贊助方案截止日期為 2016 年 6 月 17 日。</li>
<li>填寫贊助同意書前請先跟所學會 ([email protected]) 聯絡，確認各項贊助項目之剩餘名額。</li>
<li>請確認附件開立收據可否報帳再匯款，若無法報帳，請跟所學會聯絡。</li>
<li>請於接獲匯款資訊後一周內，完成匯款程序。逾期仍未匯款者，將視同放棄贊助機會。</li>
<li>以上所有贊助方案名額有限，大會將依繳費先後順序保留相關決定權。</li>

<blockquote>若贊助廠商因其他考量需取消對本主辦單位的贊助，請於 2016年6月17日前以書面方式向主辦方提出取消申請。提出後主辦方將收取給付總金額 50%之活動行政費。若在 2016年6月17日後向主辦單位提出取消申請，則主辦單位將不予退款。</blockquote>
