changes from HTN's fork:

send a slack message (email as fallback) once a request is approved
add a button in a user's profile that lets you send a slack message to them
disable editing of user's profile and remove display of phone / location
fix bug - properly reload page when some buttons are clicked
login integration. Someone who's an admin on root will also be an admin on cog. Only admitted applicants can actually use cog.
