add "DesignDiscussion" to documentation

there is currently no appropriate location in the docs where we can discuss design ideas or approaches.

An example where such discussion is needed are issues #175 and #174, where different stakeholders have different opinions on how to treat subjects - and github issues are imho not the appropriate location for that...
