create one-off page for uploading a CSV and being able to use catcorr.js #23

Not sure what makes the most sense, but one possibility would be to add this to the gh-pages branch as a standalone page and use local storage as a database backend in this situation. I haven't looked into this at all yet, but it could be a great way for IDEO-ers (and others) to quickly be able to use catcorr.

Note: Before I forget, the tricky bit with this will be giving users the ability to select the set of questions they want to include in the visualization. Free-form text answers are not particularly well suited for catcorrjs so there needs to be a way to quickly filter down to the set of questions the user wants to include.
