Create a new issue template to track our build and release process

Each build will have a primary engineer (a rotating duty) assigned who manages that build through the entire process of testing and release.

This template should serve as a checklist for the necessary steps, a place to aggregate feedback, questions, metrics, and a record of our decisions. The template should always contain the current version of our process instructions.
