I am managing a domain through Freenom, the registrar for .cf, .ga, .tk, and .ml.
They do not allow wildcard CNAME entries, so I was only able to add the CNAME entry for www as well as the A record.
The instructions are to log in and go to client area -> Services -> My Domains -> [Manage Domain for the domain you are going to use with surge] -> Manage Freenom DNS -> And then:
Add a Blank A record which points at 45.55.110.124, and individual CNAME records for any subdomains you would like, pointed at na-west1.surge.sh.
Note that '@' and '*' are not acceptable CNAME record names on Freenom.
