fix a few spacings to meet the latest ETD requirements as of 04/04/2022

This version corrects the spacings on:

• the title page (in between graduation date and "Dissertation directed by")
• the list of tables page (2 single lines spaces after the page header)
• the list of tables page (2 single lines spaces after the page header)

to meet the latest requirements from ETD, as of 04/04/2022.
