Turned on the Validator - had to fix some stuff

As the title says - had to fix some stuff.

• Added extra bindings where we put in alternate coding entries
• Corrected many cardinality settings
• Defined all the meta data extensions from l4h-templates
  etc.

Pre-Publication Tidy Ups

• Added webPASStatus and webPASPriority to referral extension elements
• Added Flag Category ConceptMap

All ConceptMaps status == active

• Fixed validation error in Ethnicity ConceptMap
• Switched status on all ConceptMaps to active

Ethnicity and NZ Residency ConceptMaps

• Added ethnicity level2 to level4 ConceptMap
• Added NZ Residency visa class codes to NZ Residency Yes/No ConceptMap

Language ConceptMap

Added Language ConceptMap for translating WebPAS 3-char language codes to proper ietf:bcp:47 anguage codes.

First Publication

Initial publication to get developers working. This is still very much a Work In Progress version; hence the version number is v0.2.0
