Removing webpack-isomorphic-tools. Because it is considered deprecated.
Using css-modules-require-hook to hook styles require for SSR.
Using asset-require-hook to hook images / fonts require for SSR.
Using assets-webpack-plugin to generate styles / scripts with hash for SSR.
